C-section scar laser treatment is not a treatment that erases scars without a trace, but rather a treatment that gradually improves redness, thickness, texture, and pigmentation. For red scars, a laser targeting blood vessels is mainly used, and for thick, uneven scars, a fractional laser that rearranges collagen is used. It usually does not end with a single session and is repeated 3 to 6 times or more at intervals of 4 to 8 weeks, with the plan varying depending on the scar type and skin tone.

The horizontal line left below the abdomen after childbirth. It is red and prominent at first, but over time, it settles down to white or brown. Depending on what state this scar is currently in, the necessary approach will be completely different. Let's look at them one by one by type below.

🩹 C-Section Scars, Why Do They Differ from Person to Person?

Most C-sections are performed using the Pfannenstiel incision method, which opens horizontally above the pubic bone, and the length of the incision is usually around 10 to 15 cm. Even with the same surgery method, the way scars remain varies greatly.

The skin continues to change even after the wound has healed. This scar maturation (remodeling) period lasts about 12 to 18 months, or up to 2 years or more. So, even if you do not like the scar right now, some parts will naturally fade over time, while other parts will harden if left neglected.

When looking at scars, they are commonly divided like this:

  • Red Scars: Early scars that look red due to many blood vessels

  • Brown Pigmentation: Darkened state after irritation or inflammation

  • Hypertrophic Scars: Red, hard, raised scars within the incision line

What is important is the 'current tissue state' rather than the time the scar occurred. The treatment direction is determined by whether it is red and active, hard and rigid, or sunken.

**PDL (Pulsed Dye Laser)** selectively targets blood vessels in red scars to calm down the color. Fractional CO₂ laser is most widely used to rearrange collagen in thick scars with uneven surfaces. While the effect is strong, it also has some downtime.

On the other hand, non-ablative fractional (1550/1927nm series) has a short downtime, enabling a quick return to daily life. Er:YAG has less thermal damage than CO₂, leading to faster recovery and is utilized for smoothing shallow scar surfaces.

⚖️ Hypertrophic Scars vs. Keloids, the Approaches Differ

The two most concerning issues with C-section scars are hypertrophic scars and keloids. Although they look similar on the outside, their natures are different.

Hypertrophic scars rise red and hard inside the incision line. Keloids grow beyond the boundary of the incision line and can be accompanied by itching or pain. If the scar grows larger and redder over time, the possibility of a keloid should be suspected.

For both types, combination therapy is much more effective than laser alone. This includes intralesional steroid injections to reduce scar thickness and itching, highly evidence-supported silicone gel or silicone sheets, and compression therapy.

Particularly, keloids have frequent recurrences, with recurrence rates reported at over 50% depending on the therapy when treated alone. Therefore, they should be approached as a management concept to consistently suppress activity and stabilize rather than a concept of removing them at once.

This is the most frequently asked question. To summarize starting from the onset time:

Laser treatment begins after the wound is completely healed and there is no oozing or infection. A few weeks to months after suture removal is a common standard. Recently, increasing research reports show that early intervention helps prevent hypertrophic scars and improve the final result.

The number of sessions is typically 3 to 6 times or more, and the interval is generally 4 to 8 weeks. Rather than expecting a dramatic change in one or two sessions, it is more appropriate to think of taking step-by-step processes where the scar fades and softens.

What about old scars? The current tissue state is a more important standard than the time of occurrence, so even for scars that are over a year old, we can set a direction after looking at their condition.

☀️ Pigmentation and Aftercare, This is Where Results Diverge

The most common worry after laser treatment is **post-inflammatory hyperpigmentation (PIH)**. Especially, the risk is relatively higher in dark skin tones like Fitzpatrick types IV to VI. Therefore, it is important to understand the skin type first and carefully determine the settings.

The core of management is UV protection.

  • Avoid strong UV rays and tanning for 4 weeks before the procedure

  • After the procedure, use sunscreen of SPF 30 to 50 or higher throughout the treatment period

  • When sunscreen alone is insufficient, combine physical blocking such as clothing and bands

Lifestyle habits also play a part. Friction from underwear bands, leggings, and abdominal binders can worsen pigmentation or hypertrophic scars, so it is better to avoid tight clothing until the scar stabilizes.

The comparison before and after treatment is viewed not by feeling but by a scale. We objectively check changes in color, thickness, and texture using clinical scales like the Vancouver Scar Scale (VSS) or POSAS.

One practical warning. C-section scar concerns and postpartum abdominal changes (stretch marks, sagging, etc.) are separate issues. It is realistic to focus plans on the scar itself rather than mixing the improvement of the scar line with the changes of the entire abdomen.
